Vision benefits continue into retirement, dental do not.

Life Insurance
Life insurance continues at the option of the retiring member, and is payable to the Association, who forwards the payment to our provider.

VSP Vision Coverage
Vision is purchased directly by FPA, and does continue into retirement. Service retirements yield 18 months of continuing coverage. Medical retirements trigger continuing coverage for 36 months.

Delta Dental
Dental is purchased for our members by the City of Fremont. Delta is not continued into retirement. Coverage terminates effective on the member’s date of employment termination.

Every employee member has coverage under Delta Dental. Their spouse, and dependents (until age 19 or 23 if a full time student) are also covered. Your dental provider should be able to look up your policy by name and likely the social security number of the FPA member named in the policy. Please download and review the attachments for more detailed information.

All FPA Employed Members are enrolled in VSP Vision Insurance under family level plans.

All FPA Employed Members are enrolled in VSP Vision Insurance under family level plans. Even those of you who are single have this coverage. Any spouses and first degree dependents (children, adopted kids, etc.) are covered.

  • Go to your vision provider and tell them you have VSP
  • Provide the FPA member’s full social security number and name
  • Identify as a first degree dependent (spouse, children, etc)

The provider should be able to access coverage based on that alone.
